Provides a wide variety of community health programs, with services including: Annual Exam/Pap Smear, Asthma Management, Behavioural Health Services, Breast Cancer Screening, Chronic Disease Care, Congestive Heart Failure Management, Diabetes Management, Doctor or clinic, Flu Shot, Gynaecological Exams, Hypertension Management, Immunizations, Insurance Enrollment, Mental Health, Optometry, Physical Exams, Primary Care, TB Tests, Urgent Care, Vision Testing Notes: We serve older adults over the age of 60 years old. If a senior is homeless or disabled, we will see the patient beginning at age 55.

Although individual nurse practitioners may be competent. We have only seen an MD once on our initial visit. My mother has been assigned another Doctor in the last few years and no one has reached out to us to make an apt to see the new doctor. Wait times are terrible (takes us hours for a visit) it seems they are always behind or short staffed. We had to wait more than 8 weeks to finally get the results of my Mom’s heart tests from Sutter. No one there communicated with Sutter nor with me about getting those results back to us without my calling all the time asking for them. The last straw is Mom has not been able to just get a simple flu shot from this clinic for 2 years. She has had to pay for it at CVS. The clinic is either out of the hi dose or requires her to be seen by a doctor even if she does not feel she needs to see one. Evidently the yearly Medicare wellness check ordered by her Doctor 3 weeks ago does not count, since it wasn’t at the over 60 clinic where her Doctor is l
